Evaluation Resources

The Division for Heart Disease and Stroke Prevention (DHDSP) has developed evaluation tools and resources to assist state health departments, tribal organizations, communities and partners in their programmatic and evaluation efforts. While many of the tools and resources were developed primarily for use by DHDSP-funded programs, they may also be of interest to entities not funded by DHDSP or working in other chronic disease areas.

Field Notes

Introducing Field Notes! Every day, we see examples of different approaches used in the field by colleagues working at the state, local, and community levels. With Field Notes, we can share some of these efforts from across the nation.

New Field Notes will be available on a regular basis, so please check in periodically, and let us know if there are specific types of efforts or activities you would like to see highlighted. • WISEWOMAN Program Evaluation Toolkit
  This evaluation toolkit was developed to provide guidance, tools, and resources to Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN) programs to conduct effective and meaningful program evaluation activities.
• Aggressively Treating Global Cardiometabolic Risk Factors to Reduce Cardiovascular Events (At Goal) Program Evaluation Summary [PDF-222K]
  Established by the Consortium for Southeastern Hypertension Control (COSEHC), the Aggressively Treating Global Cardiometabolic Risk Factors to Reduce Cardiovascular Events (AT GOAL) program was selected for evaluation as a potentially promising practice to address cardiovascular disease (CVD). AT GOAL is a practice-level quality improvement (QI) initiative that uses performance metrics, offers monitoring and feedback to practices, and provides physician education. The evaluation was conducted to describe the program’s core components, identify lessons that other programs might consider and determine its impact on intended outcomes.
